The Beach Boys PET SOUNDS promo film from '66 has them all wearing cool DON POST monster masks, including the classic "Uncle Creepy" mask! EDIT: I was able to find their track "Run Around" on Vol 8 of BACK FROM THE GRAVE by it's listed as Cavedwellers. No pic. Did a search and found this pretty funny... Great thread! Yeah the earliest I can think of would be Screamin' Jay Hawkins & The Phantom, both mentioned earlier. Joe Meek's studio band The Outlaws were sporting their western themed outfits in 1961. Not sure if you'd consider them "costumes" but since it was a gimmick of sorts I think it applies.
