Whenever I see bands playing in Europe and even South America they are always playing on nice stages with pro-lighting. Here in the US the bands are more often than not playing in front of a TV, neon beer sign, or crappy banner on some gawd-awful stage.

That's because most bands here have no following, and don't really promote themselves very well. so they get stuck playing in crappy little bars, where anybody can get a gig.
and

I think you're dead-on about this. One of the things that kept me from doing gigs for years was the frustration of dealing with that lowest common denominator. It seemed that there were dozens of bands doing the same selection of Oldies and Country, in many cases not even coming close to the original arrangement. Everything had been reduced to 3 chords even if it shouldn't have been, and the audience sat there ignoring the band and getting plastered. I sat in at a little neighborhood joint a few years ago and was amazed by how little things had changed since the '70s. It was the same songs, the same people in the audience and the same musicians; all quite a bit worse for the wear.
Since then I've sought out people that want to do the songs right and put some work into it. Even the same old same old sounds a lot better when everyone is playing the changes right, hitting the harmonies and not cluttering the songs with extra flotsam and jetsam. One thing I like about Surf is that it seems to break people out of the rut and challenge them to try something new.
Sadly, there are so many places where they are satisfied with the same old sad-arsed thing that they never even think of trying something better or promoting their venue as a place where interesting music is played.

No Jeff I don't think that is it. I really have a hard time believing The Phantom Four or The Tormentos are more popular in their country than say The Madeira in ours. I just think they have better venues and appreciate live music in other countries.

Yes people in other countries are more open to going to see music much more than here.
But...
when most people here in the USA go to see bands.
its horrible cover and tribute bands in bars.
or they go to big halls or stadiums to see mundane pop music.
because its all they know, by it being shoved down their throat.
Tom of the Surf Kings said they had a great reaction from the corwds in Germany, when they toured their back in late 90's or early 2000's
he said 1 night there was 600 or more people in this big drinking hall.
and that only about 20 of them really knew who the surf kings were, or
that they were coming. all the others were just there to hang out, and socialize and drink.
but when they started playing the whole place started going nuts.
and they did all of their originals, and covers, and had to do another set of songs they had all ready played, because the crowd wouldn't let them off the stage.
all of this on a weeknight if I recall correctly.

Just saw Big Sandy & His Fly-Rite Boys playing on a stage under two TVs which never stopped playing sports channels. The sound was down, at least. I think the stage was OK, and the lighting wasn't as bad as in most clubs, but one mic was held together with duct tape and the other seemed to be a little short on oomph. Also, they were announced several times as Big Sammy etc.
On the whole I think this was a nice deal and the band is treated well and is pleased to play there. I'm certainly happy to go see them there, ever since my more knowledgable friends put me onto it. These guys are so smooth and so professional it would be a please to hear them play almost anything. Which they do.
In this case the venue is a gambling casino up in Central City west of Denver, not a bar in town. Shows are actually free, when I'd happily pay to see these guys play. I gather a lot of people do a bit (or even a lot) of gambling while they are there, so it makes sense for the casino to sponsor bands like this. But a few details do tend to get overlooked. The same ones that always tend to get overlooked, apparently, at American venues.
It could be argued that a place's business is gambling (or drinking or whatever), not music, but it wouldn't take much savvy or even much money to fix these little things. It looks like lack of respect, but I think it's more lack of attention to detail or lack of sensitivity. Just have the bartenders flip the damn TV off during sets, fix the mics and tell the announcers the name of the band. But how many American houses have the TV on all the time. Probably a lot of people don't even notice it. I find it distracting, but others may not.

As far as lights, I only started noticing them when I started taking pictures, and since my interest is the pictures my preferences in lighting might not be exactly what a band would like for effect, but I can vouch for bar lighting tending to be dreadful in both respects. Low, absent, strangely positioned and strangely colored. Often there is a neon sign or a red spot more or less directly behind the drummer. I was at a show recently where another photographer a lot pushier than I am went up and asked the band about the lighting and came back and told me "The band says we can fix the lighting as far as they're concerned." He starting firing up some spots that were lying around untended. It definitely helped. I think the sound guys there were pretty attentive (though the sound there is always lousy due to acoustics) but they ignore lighting. I have a special color modification filter I call "strange lighting at the X" for dealing with this place.
